The Connecting the Dots through Culture team are seeking permission to take photos, use existing photos and video footage for use in our websites, and other publications, publicity and documentation as discussed with Connecting the Dots through Culture projects. I hereby Consent that an image of me may be photographed and or filmed by the Connecting the Dots through Culture team.

I give them permission to use the photos or video footage that is attached to this document. I give permission to the Connecting the Dots through Culture team to use photos or video footage in the resources that they are creating and publishing. I also give permission to the Connecting the Dots through Culture team to use these photographs or video footage for promotional purposes in the publication, media activities for educational displays and in presentations at professional development courses or conferences. I understand that the coverage and nature of the media or publications will be seen by the public.
